What you will do:
Facies and manages safe hospital discharges for service users, enabling a smooth transition back home.
Conduct comprehensive assessments of individual needs, focusing on functional abilities, moving and handling, as well as housing.
Prescribe suitable equipment and recommend minor home adaptations to support individuals in maintaining independence.
Collaborate closely with staff to ensure the best outcomes for service users, fostering a team-oriented approach to address various rehabilitation needs.
To excel in this role, being registered with the Health and Care Professions Council (HCPC) is essential, along with possessing a full driving licence to provide the flexibility needed to deliver services effectively across different locations.
