Tasks & Responsibilities

1. Personal Care: Providing assistance with daily personal care tasks such as bathing, dressing, grooming, and toileting

2. Mobility Support: Helping individuals with disabilities move around, including transferring from one place to another (e.g., bed to wheelchair), using mobility aids, and maintaining safe mobility.

3. Medication Management: Administering medication according to prescribed schedules, monitoring dosage, and keeping accurate records.

4. Emotional Support: Offering emotional and psychological support, being a good listener, and providing companionship to individuals in need.

5. Daily Living Support: Assisting with household chores, meal preparation, and other daily activities to ensure a clean, safe, and comfortable living environment.

6. Health Monitoring: Observing and reporting changes in the individual's health condition to healthcare professionals or supervisors.

7. Implementing Care Plans: Following individual care plans tailored to each person's needs and goals, as well as adhering to specific guidelines and protocols.

8. Community Engagement: Assisting individuals to participate in social and community activities, fostering inclusion, and promoting independence.

9. Documentation and Reporting: Maintaining accurate records of care provided, documenting progress, changes, incidents, or any relevant information.

10. Collaboration: Collaborating with other healthcare professionals, families, caregivers, and support networks to ensure comprehensive and coordinated care.

11. Training and Development: Engaging in continuous learning, staying updated with best practices, and participating in training sessions to enhance skills and knowledge in disability support.

Qualifications & Experience

- Qualification: Cert III or IV in Disability/Aged Care/Individual Support or similar

- Experience: One year of experience as a Support Worker
