As aHomelessness Case Manager you will provide case management supportto people who are homeless, at imminent risk of homelessness orhoused but at risk of homelessness.

Whatwill a typical day look like?

- Work closely withparticipants to develop, action and review case managementplans to meet their individual needs.
- Provide practicaland personal support, crisis budgeting, information, advice,and community referrals.
- Provide initial andongoing assessment of participant needs, including riskassessments and safety planning as needed.
- Provide supportthat is proactive, persistent, reliable, practical, andcomprehensive in a manner that is respectful of the dignity andprivacy of individuals.
- Provide servicesthat maximise the participant’s capacity for independentliving.
- Advocate and assistparticipants to access housing and specialist services forexample, drug or alcohol, mental health, legal, domestic, andfamily violence.

MandatoryRequirements:

- Certificate IVqualifications in Community Services or similar.

- Astrong communicator, able to quickly build rapport andinfluence across a diverse range of people.

OtherRequirements:

- Ability to obtain aBlue Card (working with children check positive notice orexemption)
- Ability to obtain aYellow Card (disability services positive notice or exemptionnotice)
