As the After Home Hospital Care Program Lead, you’ll play a pivotal role in guiding a team of Care Coordinators to deliver high-quality, person-centred health services across a range of healthcare programs. In this full time role based in Adelaide, you’ll be responsible for operational leadership, clinical oversight, and strategic development, ensuring our services are responsive, inclusive, and aligned with best practice.

Your opportunity

- Lead and support a team of Care Coordinators
- Oversee service delivery, ensuring compliance with regulatory standards and organisational values
- Foster partnerships with stakeholders, and service users to enhance program outcomes
- Drive continuous improvement through data-informed decision-making and staff development
- Manage budgets, reporting, and resource allocation
- Identify and implement strategies to enhance financial performance and operational efficiency
- Use data-driven insights to guide actions that improve budget outcomes and service sustainability

You will bring:

- Tertiary qualifications in Nursing or Allied Health, along with current registration with AHPRA
- Significant experience as a health professional
- Proven leadership experience of at least 2 years
- Excellent communication, problem-solving, and interpersonal skills
- Ability to lead change and best practice
- Demonstrated financial acumen
- Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
- If successful, willing and able to provide proof of up-to-date vaccinations
