As a Community Physiotherapist, you'll work with a diverse range of clients, managing both acute and chronic conditions to enhance mobility, function, and independence. You'll be supported by a collaborative team that values excellence and client-focused care.
Your clients will come from government-funded programs such as Home Care Packages, Short Term Restorative Care, and the Commonwealth Home Support Programme, as well as private referrals. Unlike typical providers, we set realistic and achievable KPIs—ask us for details
Key Responsibilities:
Conduct comprehensive assessments and develop tailored treatment plans
Support clients with mobility, falls prevention, pain management, equipment needs, and more
Work flexibly within your scope to meet the needs of both Plena and community-based providers
