Patient Flow Coordinator at University Hospitals Sussex NHS Foundation Trust.

Job Overview

The role involves supporting timely and safe patient discharges from acute care, working collaboratively within the healthcare team, assessing and coordinating patient discharge needs, and supervising junior nurses.

Main Responsibilities

Key duties include providing a comprehensive service to the Trust, facilitating effective and timely discharge of patients, and participating as a member of the healthcare team to ensure safe discharge processes.

Job Responsibilities

The successful candidate will manage patient flow, ensure timely discharges, and work closely with the multidisciplinary team. They will also participate in daily board rounds, safety huddles, and update patient information on Real Time Bed State and Medway.

Person Specification

- Foundation Degree in Health and Social Care or equivalent.
- Experience working in a related clinical area or external agency.
- Evidence of continuous professional development.
- Computer literacy and previous experience in a patient flow or discharge coordinator role.

Salary: £27,485 to £30,162 per annum, pro rata.
