About the role

We are seeking a self-motivated, empathetic Community Mental Health Practitioner for casual, fixed-term, part-time, and full-time opportunities in our new Transitional Recovery Program in Rockhampton and Yeppoon. In this role, you will work collaboratively with clients, families, and carers to deliver recovery-oriented, person-centred care. You will:

- Engage clients using motivational interviewing, brief interventions, trauma-informed care, and family-inclusive practices.
- Develop and facilitate group programs that build skills and support recovery goals.
- Support families and carers through information, education, and referrals.
- Collaborate with community services to ensure coordinated and integrated support.
- Contribute to the day-to-day operations of the service, including housekeeping duties.

This role requires flexibility to work a 24/7 rotating roster, including night shifts, weekdays, weekends, and sleepovers.

The Transitional Recovery Program supports adults leaving acute mental health services to build independence, prevent hospital readmission, and thrive in the community. In a homelike setting with 24/7 staff, we provide tailored one-to-one support, shared activities, and group programs, all built around each person’s strengths and goals.

What you’ll bring

- Tertiary qualifications (minimum Certificate IV) in Youth Work, Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
- Experience working with people with mental health challenges and complex needs, including families and carers.
- Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
- Current First Aid and CPR certificates.
- Ability to provide proof of vaccination against vaccine preventable diseases or willingness to obtain required vaccinations per Queensland Health partnership.
