Home Care Worker
We have a vacancy for a Home Care Worker to join our Community Support team, working in the central or south-eastern region of Melbourne.
- Deliver home-based support services for people living with HIV who need assistance to continue living independently in the community.
- Work with people in the LGBTIQ+ community who have an aged care package or are participants in the National Disability Insurance Scheme (NDIS) and need help at home or assistance with community participation.
Role responsibilities include attending client homes, completing activities specified in each client’s support plan, and providing assistance with shopping, cooking, cleaning, and community engagement.
LGBTIQ+ Specialist Mental Health Clinician
We are seeking a skilled and compassionate LGBTIQ+ Specialist Mental Health Clinician to join our team in Melton.
- Provide comprehensive mental health assessments and evidence-based interventions.
- Deliver therapeutic care tailored to the unique needs of LGBTIQ+ clients and their families.
- Collaborate with peer practitioners and lived experience staff to create affirming services.
Men’s Behaviour Change Practitioner
We are currently seeking experienced and motivated Practitioners to join our group of facilitators within our Therapeutic Services team.
- Conduct professional, comprehensive assessments for MBCP participants and LGBTIQA+ individuals who have used violence.
- Facilitate evidence-based group interventions focused on behaviour change for LGBTIQA+ people who use violence in intimate or family relationships.
- Collaborate with Family Safety Contact Workers to assess risk and develop safety plans.
Client Care & Support Worker
We are seeking a compassionate and community-minded Client Care & Support Worker to deliver psychosocial support and care coordination at our Positive Living Centre (PLC).
- Conduct client intake and assessments.
- Develop and monitor care plans in collaboration with clients.
- Deliver psychosocial support and targeted referrals.
General Practitioner
We are seeking contractor General Practitioners/Sexual Health Physicians who are interested in working in our unique space, including Equinox and PRONTO.
- Provide primary health care services tailored to the needs of the LGBTIQ+ and HIV-positive communities.
- Offer mental health, sexual health, hormone management, and a full range of GP services.
