Mental Health Support Worker - RRR program

- Permanent Full-Time role
- Shift times are between 8:00am – 4:00pm
- Located in Tuggerah and co-located across Gosford and Wyong Mental Health units
In this role, you will work independently and as part of a multidisciplinary team to support consumers with low to moderate mental health needs, helping them to avoid re‑admission or minimise the potential for hospital admission. You will engage with consumers through the structured 12‑week program, using a goal‑based recovery model. The program is adaptable, with a consumer‑led pace. Your responsibilities will include providing generalised and short‑term supports to help consumers build resilience and stay engaged with their recovery.
About you in the role
- Conduct initial assessments and engage with consumers weekly.
- Facilitate recovery planning using the Recovery STAR framework to identify goals and break down necessary support.
- Provide ongoing community‑based support with a transition‑based approach.
- Collaborate closely with external clinical teams, including the Local Health District (LHD), to provide monthly summaries, attend meetings, and adjust care plans as necessary.
- Maintain accurate documentation and records using CareLink and EMR ensuring that risk assessments, care plans, and progress notes are up to date.
About You
We are looking for a people‑focused, adaptable, and self‑motivated individual who can thrive in an off‑site, independent role. You will have strong knowledge of services available on the Central Coast and be comfortable linking consumers with these resources. Success in this role requires:
- Minimum Certificate IV in Mental Health or Community Services.
- A commitment to mental health recovery and the ability to use strengths‑based approaches to empower consumers.
- Strong interpersonal skills to engage with clients and collaborate with the Local Health District (LHD), as well as the ability to advocate.
- A good understanding of local resources and community access (highly regarded).
- Experience with administrative tasks and documentation systems.
- Familiarity with or willingness to learn about the Recovery Framework.
- Risk assessment skills to ensure client safety and well‑being.
