Agape Disability Services Pty Ltd is a registered National Disability Insurance Scheme (NDIS) provider in Australia. We offer a wide range of personalized support services for individuals with disabilities. These services include Supported Independent Living, Community Access, and Social and Community Participation, which aim to promote inclusion, connection, and active engagement within the community.
Our programs are designed to empower individuals by helping them develop essential life skills, achieve greater independence, and improve their mental and emotional well-being. Our approach is highly personalized, working closely with each participant to develop tailored plans that meet their specific needs and goals. We offer specific programs such as Support Coordination, which assists participants in understanding and implementing their NDIS plans by connecting them with appropriate service providers and managing their support arrangements.
About the Role
We are looking for Community support worker who can work to formalise a support plan or work plan and help them to engage different supports. The aim of a Support worker is to build capacity in different individuals, such as students, the elderly, those with a disability or people with mental health issues.

Position: Community Support Worker
Location of work: 10/19 Synnot Street, Werribee VIC, 3030
Employment type: Permanent, Full time
Salary Range: $70,000 - $80,000 plus Superannuation
The main duties of the position include:
Duties include but are not limited to:
• Working closely with clients to identify their needs, personalise support and ensure they are receiving appropriate services.
• Promoting and fostering independent decision-making, including choice and control.
• Coordinating and liaising with service providers and reviewing goals and jointly determining new ones with clients.
Key Attributes:
• High level of interpersonal, verbal and written communications skills
• Ability to work collaboratively within a team environment.
• Experience prescribing a range of disability equipment.
• High level of problem-solving skills.
• Ability to work independently (as required) and exercise initiative
• Commitment to providing a client focused service in a timely, consistent, co-ordinated and flexible manner.
• Ability to prepare outcome-based rehabilitation plans.
• Knowledge of appropriate current outcome-based assessment tools.
• knowledge of ongoing issues that living with a disability presents at various life stages.
Skills required:
• At least Diploma in Community services.
• A minimum of 1 year of experience as community worker.
