Nurse Practitioner – Residential Aged Care | Newcastle, NSW
Join a well-established organisation providing quality primary care services to residents in Aged Care Facilities across the Newcastle region. This is a permanent, full-time opportunity (5 days per week) for an experienced and autonomous Nurse Practitioner looking to make a real impact in aged care.
About the Role
You’ll work closely with the organisation’s Senior Medical Officer and General Practitioners to deliver high-quality, patient-centred care and reduce unnecessary hospital admissions. You’ll manage your own caseload, visiting assigned RACFs weekly and coordinating care between residents, families, GPs, and facility staff.
Key Responsibilities
- Conduct clinical assessments, care plans, and health reviews for aged care residents
- Undertake examinations, order investigations, and implement management plans
- Provide preventive and primary health care
- Coordinate with GPs and aged care staff to ensure continuity of care
- Contribute to medication reviews and annual health assessments
- Maintain documentation and collaborate closely with the care team
About You
- Current AHPRA registration and Nurse Practitioner endorsement
- Eligible for a Medicare Provider Number
- Strong clinical judgement and ability to work autonomously
- Excellent communication and collaborative skills
- Professional indemnity insurance and unrestricted Australian work rights
