With Mercy, you’ll have the chance to make an impact and gain experience that’s hard to find elsewhere.**At Mercy Health, it’s all about taking what we have and making it better.****To do that, we need you.****Join us at Mercy Health, and help our teams deliver even greater outcomes.****About the opportunity**The Residential In-Reach Clinical Nurse Consultant (CNC) plays a crucial role in:* Leading and responding to patient deterioration and clinical issues raised by Residential Aged Care Staff* Identifying and supporting at-risk patients through initiating proactive assessment and treatment plans* Facilitating safe hospital transfers/ bridging care for patients returning to Residential Care from Hospital* Delivering education and clinical support to multidisciplinary teams* Enhancing escalation pathways in collaboration with medical, nursing and allied health teams within Hospital in the Home (HITH), Geriatric Evaluation and Management (GEM) at Home, Community Palliative Care and the Hospital**What you’ll bring*** You’re resilient, compassionate and have a whole lot of heart.
