About the Opportunity

The Care Partner Support Officer (CPSO) plays a vital role in providing comprehensive administrative and coordination support to the Care Partner team while managing a client caseload under the Wellness Model of Care.

The CPSO serves as the primary point of contact, delivering exceptional customer service while facilitating effective communication between clients, advocates, and the care team.

A key aspect of the role involves monitoring client wellbeing, escalating concerns to Senior Care Partners when necessary, and maintaining high-quality documentation standards within specified timeframes

About You

We are seeking a committed and compassionate individual who is sensitive to the diverse needs of older people from various social, economic, and cultural backgrounds.

The ideal candidate will excel at building rapport with clients and their families, embodying qualities of kindness, enthusiasm, and energy. You should be ready to empower others while collaborating seamlessly with colleagues to achieve the best outcomes for our customers.

We seek a detail-oriented professional with strong administrative skills and excellent time management who can effectively handle multiple tasks while maintaining accuracy and meeting deadlines.

- Diploma in Community Services Coordination or Certificate III in Aged Care or equivalent
- National Police check
- Australian Driver’s licence
