- Location - Mid North Coast (Travel required within areas such as Taree and Forster)
Flexible hours and days – Monday to Friday, part-time 3 days per week; no shift work
Pay rate – $54.47 per hour + superannuation
7 additional leave days per year
Pay less tax with Not For Profit Salary Packaging
- Travel will be reimbursed at 0.88 cents per km in line with our travel policy

Our Aged Care Assessment Services ('ACAS') team is expanding, creating exciting opportunities ACAS roles offer flexibility and autonomy, based across metropolitan and regional locations, with local travel within each region. We have an exciting opportunity for a Clinical Aged Care Assessor to join our team.
Role Overview:
You will conduct assessments to identify and meet the complex needs of clients and refer them to appropriate aged care services under the Aged Care Act.
Daily Responsibilities include:
- Collaborating with clients, families, and multidisciplinary teams
- Working collaboratively with colleagues to promote a supportive team culture
- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
- Reviewing support plans to meet changing support needs
- Working autonomously and as part of a team to meet performance indicators within set timeframes
- Maintaining knowledge of funded and non-funded aged care services
- Contributing to continuous improvement of assessment processes and outcomes
Qualifications and Experience Needed:
- Health-related tertiary qualification (e.g., Registered Nurse, Occupational Therapist, Physiotherapist, Social Worker)
- Current unrestricted registration with AHPRA
- Australian Driver's license and access to a registered, insured vehicle
- Minimum one year of experience delivering aged care services in Australia or overseas, including to people living with dementia
- Willingness to apply for a Police Check
