Where you'll be working

Acute Care Team – Hornsby – Mental Health Services.

What you’ll be doing

Provide nursing and health services as a member of a multidisciplinary community mental health team that offers triage, assessment, treatment, ongoing case management and discharge planning services to individuals in the Northern Sydney Local Health Districts.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates must meet the following criteria:

- Current AHPRA registration for relevant discipline – Psychology, Occupational Therapist, Registered Nurse, or eligibility for membership with AASW for Social Workers.

- Demonstrated, comprehensive mental health assessment and crisis intervention experience; experience in suicide risk assessment and management.

- Experience working in community mental health and a demonstrated knowledge of the NSW Mental Health Act.

- Extensive case management/care coordination skills.

- Demonstrated ability to work within a multidisciplinary team setting.
