The role

Reporting to and supporting our Housing Transition and Support Manager the Housing Transition and Engagement Specialist is a key member of the Growth and Engagement team, responsible for vacancy coordination across a range of accommodation settings, including CareChoice Homes and Medium‑Term Accommodation. This role plays a vital part in client matching and managing CareChoice’s long‑term sales pipeline.
Working collaboratively across teams, this role is dedicated to creating positive outcomes for NDIS participants seeking their forever homes.
Strong attention to detail, stakeholder management, rapport building, problem‑solving, negotiation, and organisational skills—combined with a genuine passion for supporting people—are critical to success in this position.
About you
- Experience within the Disability or Community Services Sectors
- Strong networking and stakeholder management skills
- Excellent problem solving skills
- Sound verbal and written communication skills
- A clear, pleasant, professional and confident manner coupled with a genuine customer service focus.
- An ability to thrive in a fast‑paced customer service environment including the ability to manage multiple and competing priorities.
- Literacy and highly developed administration experience.
Ideally you will also
- Have an understanding of the National Disability Insurance Scheme.
- Be a positive role model with a 'can do' attitude.
- Be flexible and open to learning new skills.
- Enjoy being a team player.
Mandatory compliance
- Current and valid driver’s license
- NDIS Worker Screening Clearance (or willingness to obtain)
- Working with Children Check (Employee type or willingness to obtain)
- Current First Aid and CPR (or willingness to obtain)
