[High Salary] Home Independence Assistant

Our core business consists of the coordination and delivery of home support services and community care across Tasmania.
Responsibilities
- Uses a strength-based approach to assist consumers to achieve goals and remain living independently and autonomously within the community.
- Assist in the delivery of restorative focused home support plans/programs by providing and supporting restorative home support learning, guidance and direction to consumers and their carers, e.g. dressing and personal care, cooking, looking after their home/home safety, laundry, transport etc.
- Provide support with daily living tasks while assisting consumers to develop their independence by utilising tools and initiatives outlined in the reablement or restorative focused support plan.
- Monitor consumers' progress, assist in reviews where appropriate, and update consumer notes in the home record.
- Attend multi-disciplinary meetings as required and provide feedback on each consumer’s progress towards identified goals in the support plan.
- Carry out basic health and safety/risk assessments in the consumer's home, where appropriate.
- Maintain consumer privacy, confidentiality, dignity, choice, and control at all times, and advise your Team Leader immediately of any concerns e.g. safety issues, neglect, elder abuse, change in health status etc.
- Respect the consumer's individuality, equality, and diversity, and contribute to building a culturally safe environment sensitive to these needs.

Minimum Requirements
- Certificate III in Aged, Disability, Community or relevant discipline.
- Experience working in an aged, health or community setting.
- Current motor vehicle driver's licence.
- Two (2) professional referee checks - worked with candidate recently.
- National Police Certificate obtained within the last three (3) months.
- Working with Vulnerable People card.
- Current First Aid Certificate.
Highly Desired Experience
- Demonstrated understanding and experience in supporting consumers to achieve agreed goals and outcomes.
- Ability to work with limited supervision in a diverse community setting.
- Sound written and verbal communication skills including consumer documentation and record keeping.
- The ability to establish and maintain co-operative working relationships.
