About therole:

We arecurrently seeking a Transition Coach to join our SpecialisedSupport team in Rockhampton. Our Coach will support young people byproviding culturally appropriate, proactive, and practical supportas they transition from care services to adulthood.

Whatwill a typical day look like?

- Assist young peoplein finding a safe and affordable place to live.

- Support youngpeople in gaining access to legal advice whenneeded.

- Support youngpeople to access training, preparation for employment andresume writing.

- Support youngpeople to understand management of finances andentitlements.

- Identify youngperson’s goals and facilitate the transition of the youngperson into adulthood.

MandatoryRequirements:

- Certificate IV inChild, Youth and Family Intervention/Community Services orequivalent.
