**Department Summary**

The Heart Failure Service in the Department of Cardiology provides investigation and management of both inpatients and outpatients with acute and chronic heart failure or cardiomyopathies with the support of advanced heart failure options (state wide) which may include device therapy, mechanical support and transplantation. The Heart Failure Outpatient programme is designed for adult patients whose major problem is Heart Failure, either HFrEF and HFpEF. The programme offers a multidisciplinary approach to care including nurse coordinators, allied health and cardiac rehabilitation programmes specifically designed for heart failure patients.

**Position Summary**

In this role, you will aim to maximise the therapeutic benefits of medical management of these patients. These include implementing strategies to improve quality of life, cardiac function, and prevent or reduce the need for admission or readmission to hospital.

**Qualifications and Experience Required**

You will have a demonstrated cardiac clinical expertise that will enable you to plan, implement, coordinate and evaluate heart failure patients’ care in close collaboration with members of the Acute Heart Failure Service and multidisciplinary team. Advanced heart failure management skills, with significant outpatient experience is preferred. Experience in a coordinator role is preferred. Current registration with the Nursing and Midwifery Board of Australia is required. Post-graduate qualification preferred.
