**What You Will Be Doing**

The Diabetes Navigation role will be the critical anchor in assisting people with diabetes to transition from hospital level care to community and primary care services. During this transition, the Navigator will provide patients clinical diabetes assessment, intervention, education, service navigation and self-management support. The role will provide non-admitted care within a clinic, at home or via telehealth platforms to support positive health and lifestyle changes.

**What You Need**

We anticipate you will have:

- Must be a credentialed Diabetes educator for 3-5yrs experience.
- Demonstrate effective communication and interpersonal skills in the multidisciplinary context
- Ability to assess patient’s motivation and understand factors that impact patient ability to make positive change
- Ability to liaise and partner with the community and primary care sector.
- Ability to establish new services and programs.
