**ABOUT THE OPPORTUNITY**

As our new Support Coordinator / Recovery Coach, your main focus will be to work with and support participants with psychosocial disabilities in their recovery and coordinate support in line with the participant’s NDIS plan and their individual goals.

You will partner with those in your care to strengthen their ability to design, manage and, ultimately, control their support, assist them to take more control over their lives and to manage better any challenges they encounter in day-to-day living.

**RESPONSIBILITIES INCLUDE**
- Working equitably with Participants with respect to everyone’s right to dignity, privacy, independence, and respect.
- Supporting participants to implement their NDIS plan to maximise the value for money they receive from their support.
- Assisting and coaching participants on how to utilise the NDIS participant portal to make service bookings and changes, empowering participant choice and control.
- Achieving targets as identified by the Manager of Support Coordination, in line with the strategic business plan and guidelines as set out by the NDIS.
- Developing trusting, positive, and professional relationships with Participants and service providers.
- Providing information to participants and advocating when necessary to facilitate access to community services and facilities.
- Supporting participants in facilitating their independence by enabling empowerment and choice and promoting personal resilience and social inclusion.
- Supporting participants with Plan Reviews, community engagement & education
- Assisting NDIS Participants in identifying and planning strategies to achieve their goals as specified in their NDIS plans within a person-centered framework

**To be successful in this role, you will need**:

- Certificate IV in a preferred course or above - (including social work, applied science, community services, disability, psychology etc)
- Excellent interpersonal and communication skills
- Current valid Driver's License
- The right to work in Australia, Working with Vulnerable People Check with NDIS Endorsement and National Police Records Check
- NDIS Worker Orientation Module Certificate
- Evidence of COVID booster vaccination or valid medical exemption

**Job Types**: Part-time, Casual
Part-time hours: 38 per week

**Salary**: $31.62 - $44.77 per hour
