**About Aged and Rehabilitation Division**

Monash Health at Home, Aged and Rehabilitation service launched in December 2020 and is a fundamental part of the broader Aged and Rehabilitation Division. Monash Health at Home, Aged and Rehabilitation provides safe and high-quality home based subacute level of care for individuals with complex medical, functional, and often cognitive issues who require comprehensive, medically led multidisciplinary assessment and optimisation.

Monash Health at Home, Aged and Rehabilitation is seen as an alternative to hospital based subacute care, the service is designed to facilitate an earlier discharge from acute care and/or avoid subacute hospital stay entirely. Patient's within the service receive at least daily visits from a member of the multidisciplinary team and access to 24-hour telephone support. Patients may also access brokered services to assist with Activities Daily Living (ADLs) where there is no other support.

**About the Role**

Consultant Geriatrician who will provide clinical services to the expanding Monash Health at Home service. This service provides a bed substitution service for patients residing in our catchment and is being rapidly upscaled from its current 20 beds to 40 beds by June, 2023. You will work with a multidisciplinary team comprising of a Consultant Geriatrician, Advanced trainee in geriatric and rehabilitation medicine, an HMO, nursing and allied health; physiotherapy, occupational therapy, dietetics, social work and neuropsychology. You will be responsible for providing patient centred, evidence based care to patients who are appropriate to receive this care at home. Delivery of care utilises both face to face as well as telehealth options as required.
- Provide clinical leadership through active involvement in patient care
- Be available to medical colleagues for consultation and liaison as appropriate regarding patient care
- Contribute to the professional and corporate life in Victoria's largest public health service by participation in appropriate committees and educational platforms.
- Contribute to ongoing service improvement and clinical and workforce innovation to ensure Monash Health provides high quality accessible Rehabilitation Medicine services within allocated resources and meets the needs of Monash Health patients.

**What you need**
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Appropriate health professional tertiary qualifications
- Fellowship of the RACP or equivalent
