As a Career Mentor, you will work as an integral member of our Connecting Employment Hub team, partnering with our Participants by focusing on National Disability Insurance Scheme (NDIS) via School Leavers Employment Support (SLES). This diverse people-centred role supports Participants in developing their life skills and career goals to increase employment outcomes along with other community focused opportunities.

You will walk alongside our Participants on their journey to achieve their employment and life goals by engaging a holistic approach to professional and personal development and growth. Your approach to working through a range of funded, community and industry supports will provide our Participants with the practical skills building capacity and connection to active employment networks needed to support them.

Our approach supports you to work closely with a broad network of internal and external stakeholders to build sound outcomes for Connecting Futures Australia, our team and the Participants we support. Your focus is to build a team around our Participants enabling them to live their best lives.

- Have strong time management skills, feel confident managing your calendar to best engage with Participants and to work autonomously
- Have a desire to seek and participate in professional development to continually learn and grow
- Ensure work health and safety guidelines are adhered to
- Be open to provide services through varying delivery methods ranging from face to face, via digital technology, in workplaces and community settings, at Participant homes and a willingness to support young adults get their Driver’s Licence
- Be comfortable that this role provides a unique opportunity to have variety in your working day, requiring you to be agile and flexible in managing your case load
- Effective collaboration and conflict resolution capabilities
- Enjoys travelling to meet Participants in varying locations, holds a Full Drivers Licence and has access to a reliable vehicle

**Participation and Inclusion**
- Undertake assigned tasks to engage NDIS Participants to work towards their goals
- Complies with work practices to meet current requirements and adjusts to include new practice and requirements, supporting other team members
- Work with Participants to safely explore opportunities to expand the likelihood of participation and inclusion in their community
- Demonstrate competent use of Microsoft suite and willingness to use multiple web-based platforms
- Maintain documentation to provide accurate information and reports to meet program requirements
- Adapts working approaches to support the provision of services while retaining focus on what is important to each Participant within the agreed resources / budgets
- Applies experience and knowledge when discussing and agreeing on reasonable expectations for each Participant
- Relates positively with each Participant and their family
- Escalates issues appropriately and prepares relevant reports and information to support decision making
- Explores innovative and sustainable outcomes for Participants
- Gets actively involved in Team problem solving
- Identifies and reports risks and advises possible solutions
- Demonstrates initiative, and adapt work schedules to meet the changing needs of our Participants

**Operational Focus**
- Delivering career / mentoring services to NDIS Participants
- Delivery of NDIS funded employment supports, other funded programs as required
- Understanding of the NDIS and the employment framework
- As needed, work across various Connecting Futures Australia
