Ndis Support Coordinator and Psychosocial Recovery

Providence Healthcare (PHC) invests significantly in our staff and facilitates the best outcomes for NDIS Participants through a happy, knowledgeable, and supported team. If you are an existing Support Coordinator or Recovery Coach, PHC offers Full-time or Part-time position as required.
The NDIS Recovery Coach role also coordinates assistance to participants to build capacity and resilience through strong and respectful relationships to support people with psychosocial disability to live a full and contributing life utilising community, mainstream and formal NDIS supports via a participant's mental health recovery plan.
This role works collaboratively with participants, families, carers, and other services to identify, plan, design, and coordinate support. This is a position focusing on quality service provision plus the meeting of high-efficiency targets.

**Selection Criteria**
**Support Coordinator Role**
- A positive outlook, a quick mind and a proactive attitude
- NDIS sector experience in each role
- Have at least 2 years' experience in a Support Coordination (case management role working with people with disabilities, working with their families and allies support services, to utilizing Recovery Oriented/person centred principles to gain their independent
- Experience within and understanding of relevant mainstream and community support services.
**Recovery Coach role**:
- A tertiary qualification in peer work or mental health (minimum of Certificate IV in Mental Health Peer Work, Certificate IV in community service or Certificate IV in Mental Health) or equivalent training and a lived and learnt experience of mental health; Diploma qualification is also advantage and experiences in the community sectors.
- Demonstrated ability to operate within organizational and legislative guidelines, procedures and policies.
- Evidence of a secure home workspace
**In general, we're looking for**
- Minimum 2 years' experience in a similar NDIS or disability support coordination or social worker role
- Experience working with targets/KPIs
- Relevant tertiary qualification in a human services or allied health discipline
- Proven experience in a similar NDIS coordination or case management role
- Strong understanding of the NDIS and its processes, as well as a deep commitment to the principles of choice and control
- Previous experience working with, and a thorough understanding of, the NDIS and the ability to understand facets of NDIS Plans/Budgets
- Excellent communication and interpersonal skills, with the ability to advocate effectively
- Demonstrated understanding of the NDIS and the unique needs of people with disabilities
- Strong organisational and time management skills, with the ability to prioritize and multitask
- A genuine passion for supporting individuals to achieve their goals and live independently
**Essentials for both roles**
- Demonstrated ability to work flexibly, independently as well as a part of a team.
- Demonstrated ability to independently plan, manage and organize own workload in a complex environment.
- Victorian Driver’s Licence.
- NDIS Worker Screening Clearance.
- Working With Children’s Check.
- Current Police Check (National Police Certificate) for employment
- Proof of Identity & Right to Work in Australia.
