Primal Care Support Services is a registered NDIS provider, providing disability services to people across Melbourne. We have a wonderful opportunity for Mental health/disability Support Workers to facilitate a range of care needs for our clients as well as career growth for our staff.
We are currently urgently seeking staff to work with our clients in the NORTHERN SUBURBS OF MELBOURNE.
**Full Job Description**
This is an opportunity to join the Primal care services team supporting NDIS individuals living with a psychosocial and physical disability as well as mental health issues to participate and engage in social and community activities and achieve their NDIS goals.
We are seeking energetic, passionate, and highly committed individuals to be part of our growing team and We currently have a role requiring staff to support our participants around Melbourne in the Northern and Western suburbs including Kew and Shifts will vary according to availability.
**You will also have the opportunity to build up your career experience with our participants and ongoing professional development.**
Our participants have various support needs, and depending on the participant, your role as a Mental Health Support Worker may involve assistance to access the community, group activities, companionship, maintaining relationships and transportation and we aim to deliver high quality, person-centred services to meet the needs of our participants.

**Duties and responsibilities of a Mental Health Support Worker**:
- Provide support to participants in line with your scope of practise and NDIS guidelines.
- Empower participants to make independent choices in their lifestyle activities to meet their daily living needs as well as attain social participation in their community.
- Provide focused interventions, support and guidance to participants.
- Support and assist participants to achieve their goals as per their NDIS funded supports.
- Deliver high quality, person-centred services to meet the needs of participants.
- Recreational activities and community access
- Support to attend appointments.
- Support with meal planning, grocery shopping, making healthy life choices
- Support to research and plan activities of interest
- Support to complete administration tasks and engaging with government bodies
- As part of this role, you may be required to transport the participant with your own vehicle.
- ALSO:
Build rapport and collaborate effectively with staff/management, participants, their family and friends to achieve a positive outcome and ensure a smooth flow of service delivery throughout the course of the participants plan.
- Assist with liaising with participants family and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide a medium of strong communication.
- Increase participants skills with your active support and promote independence by enhancing their skills, and personal development.
- Provide regular and frequent quality and professional support in line with participants plan, needs, and wants.
- Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle.
- Implement strategies and programs to develop/enhance participant skills, identify areas of increased support, and opportunities for services to help achieve their goals.
**QUALIFICATIONS AND EXPERIENCE**
- NDIS Worker Screening Check
- Working with Children Check
- NDIS Worker Orientation Module Certificate (if you don’t have this already, you may do this after applying)
- NDIS Worker Induction Module Certificate (if you don’t have this already, you may do this after applying)
- NDIS Enjoying Quality Safe Meals Certificate (if you don’t have this already,you may do this after applying)
- COVID-19 Infection Control Certificate (if you don’t have this already, you may do this after applying)
- Drivers Licence (without restrictions)
- Comprehensive car insurance
- Car Registration
- First Aid & CPR Certificates
- Passport /VISA if Passport is foreign
- Certificate IV in Mental Health is desirable but not required
- Up to date Resume including you availability
- Ability to work autonomously
- Desired: Certificate IV or degree in mental health or relevant field
- Desired: Experience in a support worker role or relevant field
