Limelight People are currently supporting a growing service in the community looking to increase headcount in their established multi-disciplinary team.

**Essential Criteria**
- Previous experience of carrying a complex caseload in Community Adult Services and a mental health setting
- Working knowledge in NDIS desired
- Open to working from home but travelling for site MDT Meetings
- Qualifications such as - Diploma in Community Services, Social Work or Nursing essential.
- Excellent case management and care coordination skills.
- Community Development and program management skills
- History of working in a multidisciplinary team and taking a collaborative approach
- Current working with children’s check and police check
- Full driver’s license and access to own car
