**About the role**

The role is responsible for working within HammondCare’s Residential Care Services to coordinate and support the AN-ACC classification process for all residents to ensure an accurate care subsidy is achieved.

**_ The role is offered on a Permanent Full-time basis and can be based at any of HammondCare's Residential locations around Sydney / NSW._**

**Key Focus Areas**
- Coordination of AN-ACC processes to meet the agreed business outcomes
- Supporting Residential Care Services with achieving the best AN-ACC outcomes, assisting with the implementation of efficient and effective processes for AN-ACC related tasks
- Ensuring all claims are entered into the ‘My Aged Care Portal’ as soon as they are identified.

- Great understanding of AN-ACC, assessment and care planning development, goal setting and evaluation documentation

- Excellent organisational and time management skills
