**We have an exciting Community Support Worker role available on a **Full-time max-term basis until September 5th 2024 ( Maternity Cover) **in our Inclusion Services office based in Liverpool and servicing Bankstown, Liverpool and Fairfield area**.**
**Working days - Monday to Sunday on a rotating roster between 6am and 8pm. Maybe be required to work outside these from time to time.**
Reporting to the Team Leader, you will provide exceptional person centred support to our customers and enable them to achieve their identified goals and improve their wellbeing.

The position involves shift work, and you will need to be available to work flexible shifts that may include a combination of mornings, evenings, and weekends.

**Key responsibilities**
- Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date on a daily basis using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey

**You'll also need**
- Technical literacy with proven ability to use Microsoft Office and systems for reporting
- Minimum Cert IV in Mental Health or Disability or equivalent industry experience
- Availability to work across a rotating roster
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans. Willingness to obtain an Australian Federal Police Check, WWCC and NDIS WC to meet New Horizons employment criteria prior to commencing with us
