We have an exciting opportunity for a Team Coordinator for Accommodation services (TCA) to join our team on a permanent role based in Villa wood, NSW.

**About the Role**

In this role, you will be working within a transdisciplinary team and will be responsible for ensuring the provision of a Disability Accommodations services to the NDIS participants (Adults and children) enrolled in our services. Some of the key responsibilities of this role are:

- Lead a team of Support Workers delivering quality, quantity and timeliness of services to provide and deliver comprehensive services to participants.
- Developing, Implementing and reviewing NDIS Plans within the context of the NDIS framework, mainstream and community services.
- Identify suitable services and collaborate with participants to achieve their goal by ensuring a person-centered approach aiming at maximizing the living, social and recreational skills of people with a disability.
- Establish and maintain collaborative relationships with government and non-government entities, service providers, allied health professionals, and practitioners.
- Empower participants to develop self-direction, self-advocacy skills, collaborate effectively with professionals and other support networks, and autonomously manage their lives and NDIS plans according to their preferences.
- Generate necessary NDIS reports, encompassing progress updates and end-of-plan summaries through a comprehensive client information system.
- conducting client interviews and determining the relevant assistance needed and maximize their independence relating to the supports provided
- Preparing submissions for funding and resources, and reports to government bodies and other agencies
- Adhere to FCG policies, legislation and codes of conduct.

**About you**

To be successful in this role, you will need:

- Associate Degree, Advanced Diploma or Diploma in Social Work, Psychology, Human Services, Law or a related field.
- Minimum of 2 years of experience in disability services or a related field.
- Desirable experience in NDIS interaction with both internal teams and external stakeholders to ensure the effective placement and support of NDIS participants
- Current First Aid and CPR certificate
- Current certificate on Administer and monitor medications preferred
- A sound knowledge of Disability services and NDIS standards
- Experience working within a team environment implementing transdisciplinary environment
- A genuine commitment to supporting our participants in their home and within the community

Pay: $85,000.00 - $92,000.00 per year
