**About the role**

**The Mater at Home** service has an opportunity for a highly motivated and team-orientated **Dietitian**. This position will provide a comprehensive, high quality, evidence-based Dietetics services to community-dwelling adult clients The successful applicant will be responsible for providing a home-based dietetics service for Mater at Home patients located within a 35km radius of Mater’s south Brisbane campus. **The position is 38 hours per week over 5 days. This is an initial 3 month maximum term contract with potential for extension.**

Mater at Home provides community-based integrated services to patients along the continuum of care, from aged care to young with disability.

**Salary range: $37.14 - $56.12 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900**

**What you’ll be doing**
- Independently manage an allocated clinical workload to deliver professional level Nutrition and Dietetic services community-based adult clients within a multidisciplinary team environment, in accordance with professional and ethical standards
- Undertake nutrition assessment, implementation and evaluation tasks within well-established clinical and departmental protocols, policy and procedures, utilising evidence based practice principles.
- Demonstrated ability to participate effectively in multidisciplinary teams, working cooperatively and effectively within the team and with patients/clients to provide high quality evidence based patient care.
- Provide a clinical and professional liaison role with the wider healthcare team
- Develop clinical practice knowledge and expertise through actively participating in professional development activities, including local professional development activities, the Dietitian’s Association of Australian Accredited Practising Dietitian (APD) program and performance appraisal and development planning.
- Manage a clinical workload within a geographical area under the supervision of a Team Leader.
- Work closely with other community care providers.

**About you**
The successful applicant will be responsible for managing their individual clinical caseload while contributing to the development of continuous quality improvement strategies and best practice models of Dietetic care.

In addition to the key skills criteria, you will also have:

- Accredited Practising Dietitian (APD) status.
- Comprehensive knowledge of Dietetics in a community setting
- A demonstrated ability to manage a wide range of adult conditions.
- A willingness to provide a varied service including home visits, clinic visits, and group workshops.
- Minimum of 1 year or equivalent clinical experience
- A current Drivers Licence, valid for use in Queensland
