We are looking for a creative, enthusiastic team player to work in our Social Club for frail/aged and people with disabilities based at Cockburn Care in Hamilton Hill. In this rewarding role you will work in a team to develop and facilitate activities for the Social Club based on person-centred and reablement principles.

Day to day the role is multi-faceted and includes:

- Transporting clients to and from the Centre and to outings in the community
- Planning and implementing culturally appropriate programs and activities
- Ensuring that each client is pursuing individual goals and maintaining dignity and independence
- Providing individual support including personal care
- Maintaining the Food Plan with Safe Food Handling

**The successful applicant will possess.**
- Cert III (min) in Aged Care / Disability or Community Services (or extensive experience in the industry)
- NDIS clearance or ability to obtain within 1 month of commencement
- Basic First Aid certificate or ability to obtain within 1 month of commencement
- LR driver’s license or willingness to obtain within 6 months of commencement
- Ability to obtain an PTD endorsement to drive the bus with the clients

**Hours**: This role is being offered 7.5 hours per week. Friday only at this stage

**Salary**: $37.97 - $41.02 per hour dependant on skills and experience.
