**About the Role**:
We are looking for a casual team member to assist with leave coverage. The Telehealth Assistant’s role is to provide regular phone calls to clients, asking specific questions which allows the client to rate their health. The Telehealth Assistant enters the client’s responses in real time into computer software, which generates flags when a client’s health is potentially deteriorating. This allows for the Care Co-ordinator to review the call and arrange an appropriate response. The Telehealth Assistant will work closely with Care Co-ordinators to ensure their clients get the help they need in a timely manner. Through regular phone calls, the Telehealth Assistant provides a consistent point of contact for the client and/or their carer.
