Lucas is a 38 year old person with Down Syndrome and Autism. Lucas lives independently in his own apartment in St Kilda. Lucas has a variety of social and recreational interests and attends a day program at Annecto, five days a week.

We have an NDIS support package.

We are looking for a Care/Support - Coordinator/Plan manager (The Manager) who will:
Coordinate support workers, prepare weekly or monthly roster. Recruit new support workers.

Manage all of Lucas medical, dental, podiatry and other health related appointments.

Lucas has had some serious hygiene issues, infected scrotum, and need hospitalisation. We recommend a weekly full body check by a qualified nurse. The current carers have been trained to examine and keep a lookout for any hygiene concerns.

Deal and manage with any issues in his apartment, repairs, new bedding etc.

Be proactive in sourcing friendships, social activities and health programs. Lucas has just been accepted into the Out and About social group.

Oversee his annual budget in accordance with his NDIS plan. Currently we have a dedicated worker, Debbie who receives all invoices and pays them.

All support workers bill us with their own ABN number. This payment is a gross hourly amount like a contractor. Support workers then do their own Super and Taxation.

We have a current rate of $50 per weekday hour and $70/$80 per after-hours and weekend hours for ‘untrained’ workers. Qualified workers receive full NDIS hourly rate.

Lucas is very fortunate that one of his long term carers Tim Taubert fondly known as Nitch lives next door to Lucas. Nitch owns his own apartment. Lucas also owns his own apartment.

We have two other carers living in the same building, Bridgette and Amber. Lucas also has another long term carer Tracey M. We also have professional carer/therapist Marcelle doing Sunday shifts. We have had a recent issue where Lucas is struggling with his relationship with his father and Nitch. Marcelle is working through this. We are also working with the Alfred Hospital CDAMS unit. Cognitive Decline and Memory Services.

Lucas’ father Greg and his partner Tracy H. have always remained active in Lucas’ life managing and participating in all his activities, holidays and relationships. However Lucas is currently not recognising Greg and to a lesser extent Nitch.

The Manager will report to Greg and seek support from Nitch.

The Manager will check in with Lucas regularly and do an occasional shift if needed. At this stage we estimate around 4 - 5 hours a week but in the first month’s extra hours per week, visiting Annecto, occasional Doctor’s appointment etc. Fee full NDIS rate.

Pay: $100.00 per hour

Expected hours: 4 - 8 per week
