Maroondah City Council has an exciting opportunity for a motivated and experienced professional to join the Aged and Disability Services team.

This position is an integral part of the Aged and Disability Team, supporting referred vulnerable residents to access services, to assist them to remain independent in their own home and community.

The Coordinator Vulnerable Persons is responsible for the review of people listed on the Vulnerable Person Register, as well as the provision of wholistic Home Support Assessments, referral, linking and information services to residents of Maroondah City Council who have support needs, particularly those with complex needs. The role also provides mentoring and guidance to colleagues to support their capacity to respond appropriately to people with complex needs.

**You will**:

- be an excellent communicator, with a positive and approachable disposition contributing to a positive organisational culture and offering comfortable engagement with our clients;
- ideally have Regional Assessment Officer (RAS) experience with My Aged Care Assessor portal, using MyGovID;
- have demonstrated experience in wholistic assessments, ideally including direct experience with hoarding/squalor;
- hold a tertiary human services qualification such as social work, allied health or related;
- have a work history that demonstrates a strong client-centred approach, with a focus on enhancing client choice, control, independence and wellness;
- enjoy the benefits of working in a multi-disciplinary team and meeting a broad range of people in the community;
- have a sound understanding of State and Federal aged care policies, systems and supports, as well as an ability to network, build partnerships and work independently and collaboratively.

You will be part of the broader Community Services department, reporting to the Coordinator Assessment.

As part of our selection process, you will be required to have pre-employment checks. (Psychometric Assessment, Medical, CrimCheck and Working with Children Check).
