Signpedia is a leading NDIS Provider dedicated to delivering exceptional services to our clients, who are of Deaf, Hard or Hearing, and/or nonverbal Auslan users and their families. Our success is built upon a driven team that works collaboratively to build a strong Deaf community that supports each other.

We are seeking an experienced Support Coordinator who is compassionate and dedicated to join our team. In this role you will be assisting NDIS participants access various support services and coordinating their care. You will have the opportunity to make a meaningful impact on their lives and be part of their journey toward greater independence and well-being.

**Support Coordinator duties**:

- Assessing clients unique needs and developing personalised support plans tailored to their goals and preferences.
- Coordinate and connect clients to a comprehensive range of support services.
- Completing service agreements, support assessments, risk assessments, support plans, consent forms and other necessary forms.
- Using a Client Management System (CMS) for client data, records and booking supports.
- Recording and reporting: progress reports, case notes, accurate and detailed timesheets, appropriately addressing conflicts of interest, and following the NDIS Practice Standards for Support Coordinators.
- monitoring and evaluating the progress of support plans, making adjustments as needed to ensure effectiveness and positive outcomes.
- collaborate with a multidisciplinary team, including healthcare professionals and social service providers, to ensure coordinated and holistic care.

**Qualifications and Skills**:

- Auslan is ESSENTIAL.
- Bachelor's degree in relevant field such as social work, psychology or human services and/or Cert IV in Disability or similar and/or equivalent knowledge and experience in working with and for people with disabilities.
- Proven experience in providing support and advocacy to individuals facing diverse challenges.
- Strong organisational and problem-solving abilities, with the capacity to manage multiple clients effectively.
- High level of literacy.
- High level of administrative skills, researching skills & time management.
- Ability to understand NDIS guidelines and regulations.
- High standards of ethical and professional behaviour.

**Job Type**: Part-time

**Salary**: $32.21 - $50.48 per hour

Schedule:

- Day shift
- Monday to Friday
- No weekends

Ability to commute/relocate:

- Burswood, WA 6100: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- NDIS Worker Screening Check (required)
- Working with Children Check (required)
