**Position Title**: HACC PYP Assessment Officer
**Classification**: XV16 - ON14
**Salary per hour (exc. super**): $39.39 - $45.76
**Hours per fortnight**:48** hours**
**Employment Status**: Permanent part-time
**Other requirements**: COVID-19 Vaccination (3 doses)

**Role Description**

The Assessment Officer for the Home and Community Care Program for Younger People (HACC PYP) completes holistic, strengths based assessments for people with disability, medical or health conditions who need assistance with their daily activities to assist them to live as independently as possible in their own homes.

**The Team**

The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region. With a range of local and regional programs supporting place-based health promotion, prevention, care and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes. The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.
