**About the role**

As an APS5 Planner, you will be responsible for undertaking a diverse range of planning activities and tasks, utilizing available information to make informed decisions where options might not always be evident. Your role will encompass procedural, clerical, administrative, and operational tasks that support and contribute to the establishment of a world-leading National initiative.

Key Responsibilities may include:

- Collaborate with team leadership to manage workflow efficiently.
- Gather information from participants, representatives, planning partners, and stakeholders to facilitate evidence-based decision-making.
- Determine appropriate funded supports for participant plans to complement formal and informal supports.
- Work closely with participants and representatives to identify current and future support options to achieve plan outcomes.
- Resolve issues and complaints related to participant plans.
- Maintain accurate participant records and task workflows through administrative and data entry tasks.
- Contribute to achieving key performance indicators and planning targets.
