**This position provides an opportunity to undertake advanced training in Palliative Medicine within a specialist Palliative Care service. The Advanced Trainee in Palliative Medicine will provide quality healthcare to patients under supervision, while developing a broad knowledge of the principles and practice of Palliative Medicine and a satisfactory level of competence in Palliative Medicine as required for Advanced Training. Terms available are Community terms 1. This can be taken as part of the Fellowship training or the Diploma with the RACP.**

**Where you'll be working and what you'll be doing**
- Hospital Consult term: Review patients as requested by other medical or surgical units, review the patient within 24 hours or as close to this as possible with an AMO from the affiliated palliative care service hours, communicate with other units the objectives of care and assist in resolving conflicts between therapeutic objectives of units. Hospital Consults rotations may also involve some inpatient work.
- Inpatient term: Daily ward rounds of every patient in the unit, fully admitting or examining all new admissions, discharge planning, together with patient and paramedical staff and family, ensuring appropriate post-discharge follow-up, supervising discharge summaries and communication with the patient’s general practitioner.
- Cancer Term: Working with Medical Oncology, Radiation Oncology and Haematology in clinics and rounds. Palliative Care clinics, rounds and consults as needed.
- Community term: Notify AMO of new referrals on day of assessment, see all new referrals within 3 working days, and within 24hrs for urgent referrals, liaise promptly with the patients general practitioner after each visit, regularly review unstable patients in collaboration with the GP, see patients in all home settings as directed (residential homes, hostels, nursing homes), liaise regularly with the GPs and Care givers as appropriate, provide systematic management of the community case load.
- Outpatient care: Attendance at unit outpatient clinics, assessment of unit patients in the emergency department.
- Participating in 24/7 rotating roster of days, evenings, nights and weekends.
- Participating in an on call roster and weekend ward rounds as rostered.

**Essential requirements**
- MBBS or equivalent, currently registered with the Medical Board of Australia in an appropriate registration category.
- Demonstrated clinical practice as a registered medical practitioner in the last 2 years in a setting appropriate to the needs of the position.
- Enrolment in a relevant advanced training program including Australasian Chapter of Palliative Medicine, RACP Palliative Medicine or other subspecialties, Clinical Diploma of Palliative Medicine (RACP), Faculty of Pain Medicine or Family Medicine Program or eligible for advanced training in Palliative Medicine by the possession of a Fellowship in another college recognised by the Chapter of Palliative Medicine.
- Valid Working with Children Check, cleared National Police Check.
- To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy, this includes any amendments made from time to time.
