About the role

We are seeking a dedicated individual for Housing Support to provide case management outreach and support to people who are homeless or at risk of homelessness to access safe, affordable accommodation and prevent their return to the homelessness service system. Additionally, case managers develop person centred and holistic case (and or safety) plans to ensure the individual needs and risks of each client is met throughout their service period with The Salvation Army Homelessness Gippsland.

Key Responsibilities
- Strong networking relationships are formed with relevant stakeholders to ensure coordination to meet the needs of homeless people
- Effective relationships are established with clients that encourage self-determination and participation
- Case management support provided to clients in accordance with TSA case management models
- All plans are regularly updated and reviewed with the client at a minimum standard of every 3 months
- Tasks identified by management team to be completed within required timeframe

This is a fixed term (6 months) full-time position, based in Leongatha, VIC. Salary and conditions are in accordance with SCHADS Level 5.

What you will have
- Relevant tertiary qualification in Social Work, Community Services, Youth Work, Psychology or Community Development - Diploma Level (minimum)
- Demonstrated knowledge of theoretical frameworks that underpin attachment theory and trauma informed practice
- Expertise in working with clients with multiple vulnerabilities, histories of trauma, and complex needs
- National Police Record Check is required
- Working with Children Check is required
- Current and valid Driver’s Licence is required
