**About the Opportunity**

We are seeking dedicated and compassionate Support Workers to assist an active, young teenager with 24/7 support on a rotating roster. You will work to assist with his daily schedule and will be responsible for assessing clients' needs, developing and implementing intervention plans, and collaborating with other professionals to ensure holistic care.

**Key Tasks Includes**:

- Conduct comprehensive assessments to identify clients' needs and challenges.
- Maintain accurate and up-to-date client records.
- House responsibilities
- Assisting in completion of a daily schedule including visits to parks, beaches and outdoor activities such as fishing, swimming and riding bikes.
- Working on a rotating roster to provide care 24 hours per day.

**Skills and Abilities**:

- Minimum Certificate in Aged Care or Disability Services
- Experience working with complex clients
- Experience with Risk Assessments and Medication Administration
- Strong communication and interpersonal skills.
- Experience in Trauma Enforced and Disability background
- Knowledge of relevant laws, regulations, and ethical standards.
- Ability to work collaboratively in a team setting.
