Be a key team member at a locally owned, small provider of home care packages who believe in supporting clients to be at home, allowing them to live their life, their way. They pride themselves on offering individually tailored services including assistance with everyday tasks, social support, home maintenance and respite care.

**The exciting opportunity**:

- Based in Perth’s Southern suburbs, with most clients in the Fremantle area
- Report directly into the Managing Director
- Coordinate the delivery of person centred home care services, promoting healthy ageing, increasing client’s ability to stay in their home and facilitating community re-engagement
- Liaise with ACAT Assessors and conduct initial visits with potential new clients
- Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
- Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options
- Provide leadership, support and mentoring to the support staff
- Be involved in identifying gaps and creating continuous improvement initiatives

**Benefits**:

- $90k - $95k + super
- Open to people without a clinical background
- Good opportunity for Service Coordinators in disability to transition their skills to home care or Home Care Support Workers to step into a Case Manager role

**About you**:

- Bilingual and able to speak English and Italian is essential
- Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
- Able to create and review care plans that match client’s needs and goals
- Committed to using a client directed model of care to empower the elderly and encourage reablement and community engagement
- A current driver’s license and the ability to manage your time and work independently
