**Your LINC** is a mental health outreach service provided by Centacare in partnership with KWY Aboriginal Corporation. We offer holistic, flexible, and culturally responsive psychosocial support for individuals experiencing mental health challenges and distress across the Adelaide Metro, Barossa, Hills, Fleurieu, Limestone Coast, and Local Health Network regions.

**The Role**

Centacare Mental Health, Wellbeing and Counselling (MHWaCS) Unit, through our Your LINC program, offers multiple full-time and part-time (0.8 FTE) positions as Peer Workers within the NALHN area. Our program provides both short-term (6 weeks) and longer-term (4-12 months) mental health outreach services to support individuals facing challenges and distress.

In this role, you'll use your lived experience to support individuals facing mental health challenges, along with their families and carers. Your responsibilities will include promoting well-being through recovery-oriented activities, facilitating smooth transitions to appropriate services, and providing support for individuals' transitions and exits with appropriate referrals.

Other responsibilities
- Advocate for individuals by representing their interests and empowering them to advocate for themselves when necessary.
- Utilize knowledge of the NDIS to assist clients in navigating eligibility criteria, accessing services, and facilitating transitions.
- Establish and maintain relationships with local support networks to help clients integrate into their communities and lead fulfilling lives.
- Maintain accurate case notes and efficiently utilize data management systems to track client progress and outcomes.
- Understand the impact of mental health challenges, provide tailored support to individuals experiencing distress and diverse needs, and support carers by recognizing their role, offering timely information, and facilitating access to additional support as necessary.

**About You**

As an experienced Peer support worker with lived experience in mental health recovery, you excel in providing valuable support to individuals facing mental health challenges. You have strong interpersonal skills and ability to make sound decisions under pressure build trust with stakeholders and effectively manage crises. Your commitment to excellent customer service ensures prompt and efficient meetings of client needs through careful planning and prioritization.

Qualifications & Experience:

- Certificate 4 level qualification in Mental Health Peer Work.
- Lived experience of mental health challenges and recovery.
- Experience and expertise in working directly with people with mental health issues, complex needs, and with their families and carers.
- Proven ability to work autonomously and as a member of a team.
- Demonstrated ability to plan and prioritise to meet client service delivery requirements.
- Demonstrated experience in documenting client case notes and working with electronic data bases.
- Demonstrated understanding of available community services, networks and supports.
- Great interpersonal and listening skills to build strong relationships with key stakeholders.
- Excellent customer service skills.
- Demonstrated experience working with the recovery framework and taking a trauma informed approach to practice.
- Ability to make sound decisions under pressure and de-escalate crises.
- Valid Department of Human Services (DHS) Working with Children Check.

**Desirable**
- Degree in relevant Human Services discipline.
- Experience using Penelope electronic client management system.
