We are seeking an experienced Case Manager who ideally has a background as Registered Nurse/Allied Health Therapist with current APHRA registration or Social Work, to join our multi
- disciplinary team. This role is being offered on a permanent **full-time** basis to support clients in and around the **Metro North East** region. The Case Manager is responsible for care coordination for community clients within a holistic reablement and wellness approach to enhance the lifestyle and living experiences of those residing at home. The Case Manager will participate in the implementation of a client directed model of care which focusses on empowerment of the client to maximise quality of life, promote healthy active ageing and facilitate community re-engagement.
To be successful in the role, we are seeking someone that has come from a **clinical/allied health** background and who has knowledge of the New Aged Care Standards and an understanding and experience in the management of Home Care Packages. The applicant must be innovative and committed to driving a culture of excellence and quality service to Bethanie’s clients.
**Key Responsibilities**:
- Provide professional and competent delivery of services to clients in line with the New Aged Care Standards, Commonwealth Home Support Program and Home Care Package legislation
- Client and goal centred approach to care planning, incorporating regular and structured review processes to ensure currency and appropriateness of care plans is maintained
- Develops sustainable relationships with local networks and external agencies to strengthen access to social opportunities and lifestyle options across the service footprint
- Responsible for ensuring all client budgets are monitored and managed in line with approved package level and individual client requirements
- Liaises multi-disciplinary team members to ensure effective and holistic care is delivered to all clients. Links include, but are not limited to: Clinical and Registered Nurses, Dementia Consultant, Occupational Therapist and Physiotherapist
- Provides reports, information and support as requested by the Regional Community Manager to a highly quality and in accordance to policy and procedures
**Requirements**:
- Willingness to uphold the Mission, Vision and Values of The Bethanie Group Inc.
- Knowledge of the Home Care Packages Programs and Community Care Common Standards.
- Demonstrated experience in the Aged Care arena
- Experience managing a team of care providers
- Excellent customer service, negotiation, communication and support skills
- Valid driver’s licence and access to a reliable vehicle
