Your Opportunity

The Reconnect Youth and Family Worker provides flexible case management to clients in locations accessible to the client, including schools, community settings and family homes.

Reconnect is a community-based program supporting young people who are experiencing or at risk of becoming homeless or experiencing family conflict. Reconnect aims to prevent homelessness by linking with accommodation options, counselling, mediation and other supports.

Your key responsibilities will be to:

- Working collaboratively with wellbeing team in state high school to develop a Support Plan with the client, the worker aims to enable improved family relationships and school engagements
- Connecting with student referred to service and triage with wellbeing team for suitability
- Maintain participation in education or vocational settings, and engagement in the community
- Provide person-centred case management
- Respond to referrals of clients to the service from internal and external support services
- Facilitate group programs to students in school
- Build and maintain positive internal and external relationships

Requirements for Success
- Relevant degree or tertiary qualification with at least 2-3 years’ experience
- Demonstrated professional approach to the role including maintaining professional boundaries.
- Demonstrated ability to provide specialised or complex therapeutic interventions and manage a caseload of complex client.
- Experience managing internal and external stakeholders.
- Well-developed written and verbal communication skills.
- Highly developed administrative skills and the ability to provide support to management of a complex nature.

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
