On behalf of our client which is a well established Health Service in regional Western Australia in the Central Wheatbelt which is three hours drive from Perth, we are currently seeking an experienced Social Worker to join a supportive Allied Health Team for a locum contract. The role is fulltime Monday to Friday role for a 12 week period with possible extension. In this role you will be responsible for providing a high quality, direct social work service to all inpatients and outpatients of the Hospital as required. This includes counselling, access to support services, discharge planning and community follow-up.

**Key Responsibilities**
- Provides social work services for clients referred to the service
- Assess, deliver and evaluate client care appropriately, coordinating community referrals
- Coordinate and promote holistic approach to care for families with complex social issues.
- Coordinate multidisciplinary communications for delivery of care to complex families.
- Provide client and carer information and support, utilising and promoting all other appropriate agencies.
- Provides consultancy and advice on social work and related issues to medical, nursing, administrative, allied health and other staff.
- Liaises with key stakeholders in relation to the areas of social work and social and emotional well-being in the community.

**About You**
- Completed Bachelor or Masters in Social work
- Effective communication and interpersonal skills
- Ability to work collaboratively and effectively in a multidisciplinary team
- Demonstrate initiative and creativity in thinking
- Be willing to contribute to the development & promotion of a cohesive & interactive team
- Act as a professional role model when dealing with internal and external customers
- Willing to participate in a continuous quality improvement environment

**Agency Rates**
Up to $53 p/h + Super
- based on experience + Share Accommodation.
