Your role at St John of God Social Outreach

We are looking to hire a Social Worker (Grade 2) on a permanent basis to join our Residential Aged Care Program in Bendigo.

As a Social Worker, you will work alongside the multi-disciplinary team to provide social work services to consumers and ensures practice is in accordance with the Australian Association of Social Workers_ Code of Ethics and Practice Guidelines_ and SJGHC policies and guidelines.

You will be expected to work independently and demonstrate a specialist level of knowledge and depth of experience within aged care visiting residential facilities.

The Position
- Provide evidence based senior Social Work services to consumers/carers within the clinical specialty area/s.
- Provide Social Work assessment, intervention and evaluation at an advanced practice level for complex and general caseload/s.
- Ensures prioritisation and delegation of duties for own caseload.
- Contributes to the evaluation of the Social Work Service and Clinical Specialty, to ensure continuous improvement.
- Ensures effective multidisciplinary teamwork through professional communication and comprehensive liaison with colleagues, consumers/carers and service providers.
- Ensures maintenance of appropriate clinical documentation and clinical information systems.
- Demonstrates reflective practice and commitment to professional development.
- Supports Social Work service through communicating issues and ideas for enhancing the service (through to the coordinator of Social Work).

You will have clinical experience in a specialised area of practice within a healthcare setting and develop evidence based, innovative clinical practices and initiation of quality improvement activities.

Successful applicants will have tertiary qualifications in Social Work and be eligible for membership with the Australian Association of Social Work

- Salary: $89,954 to $107,578 + 11% Superannuation
