**Position Title: Disability Team Leader**

**Purpose**: Ensure all participants are provided with appropriate staff and roster shifts as required. Provide leadership to a team of support workers to deliver a quality person centred and recovery focus for participants.

**1. Purpose of the Position**:
The Team Leader will assist with training, and support team members across the Disability Services. The position is responsible for ensuring that health and wellbeing objectives, needs and goals of each person receiving the services are met and maintained. The role will involve rostering, brokerage agreements, and other expenditure monitoring to align to budgets for each house. The role may be required to supervise lower classified staff with supervisory responsibilities for some complex work and may undertake planning and coordination of activities. The role will operate under the Accommodation Coordinator, for day-to-day supervision, and assistance sought when complications occur.

**1.** **Service Delivery and Planning**
1.1 Monitor and action all client specific documentation, including data collection and file notes as directed.
1.2 Assist with reviewing all necessary reports that will contribute to effective client support.
1.3 Assist with storage and exchange of client records to meet privacy standards and documented consent.
1.4 Preparation of fortnightly roster, to ensure adequate numbers of staff are trained and roster reported to Supervisor in timely manner. Rosters to minimise overtime, where applicable.
1.5 Rostering consistently with Work Hour agreements and contract of hours for various PPT staff
1.6 Accurate data entry on fortnightly rostering, including notes for changes, and annual leave etc
1.7 This role will also have on-call responsibilities. The role may also require responding to emergency situations outside of normal business hours when required.
1.8 Facilitate with all staff the ability to develop and work with person centred plans.

**2. Unit Organisation and Innovation**

2.1 Assist the Accommodation Coordinator with regular reports on activities and participant meetings and as directed.
2.2 Provide appropriate support to staff during their daily routines, provide advice and feedback to staff when issues arise that require clarification or assistance in a respectful manner.
2.3 Undertake the daily tasks of the accommodation service and attend to the personal and health care requirements of the resident as per support plans to ensure the efficient functioning of the home.
2.4 Liaise with the Accommodation Coordinator, to assist with completing all requested appointments for the client in a timely manner.

**3. Team Leadership**

3.1 Commitment to leading by example and getting the best out of the team through guiding and working alongside staff with the underpinning values of respect, fairness and equity
3.2 Provide support, guidance, and feedback to staff on a range of matters and keep the DBM informed of such matters in timely manner.
3.3 Attend all team meetings and supervision meetings. Organise meetings with guardian and other practitioners as appropriate or directed by management.
3.4 Develop the team by creating an environment that encourages ideas, quality improvement and reflection on performance.

**Job Type**: Part-time
Part-time hours: 32 per week

**Salary**: $34.00 - $36.00 per hour
