Central Bayside Community Health Services (CBCHS) - Social Support Group team has an exciting opportunities for compassionate and motivated Lifestyle Group Supervisor
- We have
- **part time roles** available, our programs run between
- ** 8.30am and 4.30pm, Monday to Friday** with no late nights or weekend work required. We support people with a range of abilities, from independent, and complex support needs.

We also include personalised day trips and individualised 1:1 support. The Lifestyle Group Supervisor role is a rewarding and unique opportunity to work in a small programs. This role is responsible for both planning and coordination of the Social Support Group activities that develop, maintain and support social interaction and independent living (SSG) for older persons and older persons experiencing cognitive changes. This role also provides valuable interactions, care and support to participants with the ability to build a real connection with clients.**Responsibilities**

- Client driven purposeful & stimulating activities for clients that are diverse and appropriate to the recipients of each group program
- Develop three monthly program plans and Risk Assessment Plans for the group, in accordance with the relevant policies
- Liaise with clients & carers in regard to their ongoing well-being and be responsible for monitoring and documenting clients’ activities and attendance on a regular basis
- Lead group activities including driving clients to and from their home and to community based outings.
- Closely supervise all clients in the group, ensuring their safety & wellbeing at all times.
- Liaise with carers, families and other health workers after consultation with Co-ordinator/Clinical Lead to ensure a holistic approach to support is provided.
- Direct support worker on their client specific tasks each day.
- Meet with the Wellness & Social Support Co coordinator and/or Clinical Lead quarterly to discuss the program and client list

**About you**
- Demonstrated experience working with adult clients
- Minimum qualification - Certificate III in Aged Care or equivalent.
- Certificate IV in Leisure & Health or equivalent desirable.
- Demonstrated working experience within an aged care/community health setting.
- Previous experience in similar role highly regarded.
- An understanding of the Aged Care sector, and/or experience working with Aged Care clients.
- Excellent communication and interpersonal skills, with the ability to liaise with a wide range of individuals including clients, volunteers and staff.
- Strong client focus and the ability to establish and maintain relationships with a range of participants.
- Exceptional record keeping (paper based and electronic) and database management skills.
- Ability to work autonomously and as part of a team.
- Possess strong professional ethics including the ability to exercise sound judgment and discretion in relation to sensitive matters.
- Ability to demonstrate a flexible approach that is responsive and innovative.
- Understanding and proven commitment to person centred care and client driven services.
- Demonstrated working experience within an aged care / community health setting desirable.
- Previous experience in handling client fees and program money desirable.
- Current Victorian Driver’s Licence & willingness to drive a 11 seater bus.
- Current First Aid Certificate desirable.
