**About The Role**

The NDIS service Co-coordinator role is to provide support to the NDIS participants and assist them in achieving the goals set out in their NDIS plans which will include:

- Permenat Part
- Time (3 days per week) at Templestowe Office
- Working with our clients to achieve their goals in line with the NDIS plan
- Providing ongoing support Coordination services to clients in line with their NDIS plan
- Using a coaching approach to assist our clients in capacity building and independence
- Engaging and co-coordinating service providers on behalf of clients and linking clients to relevant providers that best suit the client’s needs, including negotiating costs, arranging assessments etc
- Maintaining up to date knowledge of NDIS systems, practices and local services and organizations
- Providing crisis support to the participants
- Fostering and developing positive relationships with NDIS stakeholders
- Assisting TL HS With
- Intake/on boarding of new NDIS participants
- Follow up Referrals
- Assessments/reassessment

**To be successful for this role you will need**:

- Demonstrated understanding of how disability can impact on a person’s life
- Demonstrated knowledge and experience of NDIS Support Coordination
- Relevant experience working with people living with disabilities
- Experience coordinating specialist services for those living with complex needs and working with cross-functional teams
- Excellent project management, communication, liaison, negotiation, and problem-solving skills
- Demonstrated knowledge of the issues facing NDIS participants
- Demonstrated ability to work effectively with culturally and linguistically diverse consumers
- Demonstrated attitude of flexibility and adaptive nature to respond to changing needs of participants
- Demonstrated ability to work cooperatively as a member of a professional care management team
- Demonstrated ability to work autonomously and to manage a wide range of tasks and given timelines
- Physical ability to perform stated duties
- Ability to demonstrate and display the Arcare values
- Personal attributes:

- Able to complete tasks in the allocated time
- Able to priorities tasks
- Able to work as per Work Health & Safety requirements
- Adhere to BodeWell
- Community care Home Care policies and procedures

**Qualifications**
- Tertiary qualification in a relevant field social worker, psychology, allied health, community services, nurse, social or health science professiona
- At least two years' experience in a relevant field

**Essential requirements**:

- Valid Australian full driver’s license
- Current valid Working with Children Check
- Right to work within Australia
- NDIS Worker Screening Check
- Willing to Use Own Vehicle when Required (km paid)

**Additional Requirements**
- Proof of Certificate of COVID-19 Vaccination
- Own Vehicle
- Flu vaccination
