About the role

**Objectives of the service are noted as follows**:

- Deliver culturally safe and inclusive prevention and early intervention care people at risk of, or recently experiencing, hospitalisation for a chronic health condition (including physical and mental health concerns).
- Provide supported discharge to community through an Indigenous Hospital in the Home model and warm referral to primary care.
- Development and delivery of a service model that considers and addresses social and cultural determinants of health through localised partnerships.
- Deliver culturally safe and inclusive care for First Nations peoples in the West Moreton community that builds resilience, health literacy, and connectedness to family and community.

This service will be delivered in the community setting, through a combination of outreach and community-based clinics.
The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support to the First Nations Holistic Wellbeing Service located in the Preventative Integrated Care Service within the Preventative Health Division.

About you
- Demonstrated advanced clinical knowledge in the area of the chronic condition management delivering expert nursing care in complex situations while providing support and direction to other team members.
- Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
- Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use Virtual Care, digital health/information and reporting systems appropriate to the position.
- Demonstrated ability to communicate effectively with Aboriginal and Torres Strait Islander people and their families and communities to maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
